Novice Program (Swim lessons)

STAR Swimming is offering a Novice program (swim lesson) this year.  We will be offering the Novice programs both at Frontier Middle school on Fridays from 5:00-5:40pm and at ECC City on Tuesdays from 4:30-5:10pm.  Please click the following forms to dowload and have filled out by the beginning of the 1st lesson.

ALL Checks are made out to STAR Swimming.

1. 2011-12 Novice swim lesson program application (Session 3) - $50.00 (January-February)
2. USA Registration form (Mandatory Insurance once per year) - $58.00    
   This form is sent in with the other forms and the check is actually made out to STAR Swimming.
3. Outreach apllication - if necessary
